Monitoring and support to manage symptoms and pain, promoting comfort and wellbeing, with concerns escalated in line with clinical protocols.
Ongoing monitoring of long-term conditions
Regular review of long-term or complex health conditions to support stability, prevent deterioration, and promote effective condition management.
Liaison with GPs and other healthcare professionals
Coordinated working with GPs, community nurses, and relevant professionals to ensure safe, effective, and consistent care delivery.
